Our local authority client in East London is recruiting a Care Co-Ordinator: To provide support and coordination of Independence Assistants for short term reabling care to those in the community following hospital discharge. Includes creating and amending staff rotas, accepting referrals, informing managers re capacity.

To be responsible for the work and of 15 assistants visiting people in their homes to provide reabling support, including personal care. The job is demanding and requires the ability to work under pressure and multi-task. Supporting staff in the community and gathering feedback, including identifying and sharing risks and need for other agencies input and to provide supervision for staff including out of hours co-ordinators and participating in the standby on-call rota:

Qualification – Essential

GCSE Grade A-C (or equivalent) in English Language and Mathematics
Relevant professional level qualification
Advanced – Microsoft; Word, PowerPoint, – Excel & Outlook
Skills, Abilities & Experience – Essential

Ability to work under pressure to changing deadlines in a busy office environment
Proven experience of working to agreed quality standards, work effectively with members of the public and to resolve contentious issues within agreed frameworks
Able to communicate with experience across a broad section of service users – including customers and internal/external agencies
Ability to liaise effectively with people inside and outside the organisation, including existing and potential customers, and professional staff & family
Ability to participate in the review process with customers and to work jointly with other professionals to ensure customer’s needs are met and enhances the quality of life
Ability to keep detailed electronic records and notes including regular database updates
Evidence of consistent accuracy with repetitive work which requires good written, analytical skills
Advanced ability to work with changing IT programmes and software
Knowledge of various administrative filing and monitoring systems
Excellent ability to manage and collate sensitive data
Ability to partake in quality assurance processes including monitoring and evaluation
Able to provide Five Years of references – essential
Enhanced DBS on the update system – essential
