We are recruiting an Adult Social Care Assessor to access Adult with Physical Disability and Older People for a public sector client in South West London: To undertake assessments, support planning, identification of resources and reviews to assist in maintaining the independence of adult social care service users:

Responsibilities
To screen, gather information and assess adults with social care needs and their careers, under Adult Social Care criteria and taking into account an individual’s communication and cultural needs:

Qualification – Essential

GCSE Grade A-C (or equivalent) in English Language and Mathematics
QCF Diploma in Health and Social Care (Adults) Level 3 or NVQ Level 3 in Promoting Independence
or Health and Social Care or a willingness and ability to obtain the qualification
Advanced ECDL –Microsoft Office, Word, Excel, PowerPoint or Adobe, Outlook and Social Media
Skills, Abilities & Experience – Essential

Good understanding of adult community care activity within the statutory, voluntary or private social care sector
Knowledge of causes and effects of common disabling medical conditions
Considerable experience of working within a social care setting, working with people with disabilities or older people
Considerable experience of working with vulnerable adults in the community with experience of assessing need via assessment or review or using assessment skills to meet the current need
Able to manage a workload to agreed targets and able to use initiative and work without direct supervision
Enhanced CRB/DBS; on the disclosure & barring service update system – essential
Able to provide Five Years of references – essential
